Penn Medicine is committed to delivering exceptional patient care, advancing medical research, and educating future healthcare leaders. As a Hospice Social Worker, you will play a vital role in our interdisciplinary team, providing compassionate care and support to patients and their families.

Responsibilities:
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.
  • Collaborate with the interdisciplinary team to ensure seamless transitions in care and provide guidance on advance directives.
  • Assess and address complex biopsychosocial systems, identifying patients and families at risk and developing strategies to mitigate these risks.
  • Communicate effectively with patients, families, and healthcare professionals, preserving confidentiality and dignity.
  • Stay current with new technologies, treatment modalities, and resources, utilizing internet and other sources to enhance patient care.
Credentials:
  • LCSW or LSW licensure is required.
  • Master's degree in Social Work is required.
  • 2+ years of post-Masters experience with 1-2 years of related experience in social work or a related field.
