Job Summary

Pennsylvania Medicine is seeking a highly skilled and dedicated Per Diem Pharmacist to join our team. As a Per Diem Pharmacist, you will play a critical role in providing exceptional patient care and contributing to the success of our pharmacy department.

Key Responsibilities
  • Provide Medication Therapy Management: Review pertinent clinical information to ensure medication therapy is appropriate based on the patient's age, diagnosis, and clinical status.
  • Collaborate with Healthcare Providers: Confer with physicians and other healthcare providers to resolve discrepancies and problems with medication therapies.
  • Offer Alternative Drug Therapy: Provide alternative drug therapy to improve outcomes and/or provide the most cost-effective therapy.
  • Perform Dispensing Functions: Review written or directly entered medication orders, process medication orders, and verify medications filled by pharmacy technicians.
  • Assist with Controlled Substance Distribution: Assist with the distribution and control of controlled drug substances within the Pharmacy Department and to patient care areas.
  • Participate in Meetings and Rounds: Participate in meetings and rounds based on scheduled assignment with medical attending and house staff to discuss chosen modes of therapy and to review rationale behind such selection.
  • Provide Pharmacokinetic Monitoring: Provide pharmacokinetic monitoring when a targeted drug is prescribed for a patient.
  • Provide Consult Service: Provide consult service dealing with various aspects of drug therapy, including pharmacokinetic consults, adverse drug reaction reports, and possible drug-drug interactions.
  • Identify and Assist in the Management of Medication Errors: Identify and assist in the management of medication errors and adverse drug reactions.
  • Act as a Preceptor: Act as a preceptor for the Pharmacy Practice Resident.
  • Provide Patient Education: Provide patient education on the proper use of prescribed medication and associated compliance.
  • Support Customer Service and Performance Improvement Goals: Support customer service and performance improvement goals of the Pharmacy Department; collaborate with other caregivers/staff, within and outside the Department, to develop means of enhancing patient care and services.
  • Maintain Standards of Confidentiality: Maintain standards of confidentiality.
Requirements
  • Pharmacist License: Must be a licensed pharmacist in the state of New Jersey.
  • Pharmacotherapy Certification: Must have pharmacotherapy certification.
  • Board Certified Pharmacotherapy Specialist: Board certified pharmacotherapy specialist preferred.
  • Education or Equivalent Experience: Must have a Bachelor of Arts or Science degree and a PharmD or BS Pharmacy degree from an accredited College of Pharmacy.
