Key Responsibilities- Manage patients through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
- Communicate and collaborate with the interdisciplinary care team to ensure comprehensive care.
- Conduct advanced illness and advanced care planning conversations to support patients and their families.
- Perform comprehensive assessments and document findings in a concise and compliant manner.
- Facilitate agreement and implementation of the patient's plan of care by engaging facility staff, families, and primary and specialty care clinicians.
- Coordinate care as patients transition through different levels of care and care settings.
- Review orders and interventions for appropriateness and response to treatment to identify the most effective plan of care.
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following: Adult Nurse Practitioner, Adult-Gerontology Acute Care Nurse Practitioner, or Adult-Gerontology Primary Care Nurse Practitioner.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and dwelling conditions.
- 1+ years of clinical experience in senior living, geriatric medicine, or home care.
- Proficient computer skills, including the ability to document medical information with written and electronic medical records.
