Responsibilities
  • Conduct in-home visits to engage patients in our care model and provide comprehensive medical care
  • Partner with primary care physicians to deliver concierge-level geriatric medical care and social support
  • Collaborate with our interdisciplinary team to ensure seamless care transitions and coordination
  • Prescribe and manage medications, including psychopharmacological agents, as needed
  • Update patient records and ensure accuracy at each appointment
  • Deliver high-quality care while maintaining company metrics and adhering to regulatory requirements
Requirements
  • Master's degree in Nurse Practitioner studies from an accredited program
  • Active, unencumbered RN and NP licenses in the posted state
  • Current board certification (AANC or AANP)
  • BLS certification and active NPI# as an NP
  • Active, unencumbered DEA licensure or eligibility to apply
  • Previous experience in home care, primary care, hospice, palliative care, or geriatrics preferred
