Responsibilities- Conduct initial assessments of patients and families to determine home care needs
- Develop and implement individualized care plans to meet the unique needs of each patient
- Provide education and support to patients and families on disease management, self-care techniques, and prevention
- Collaborate with healthcare teams to ensure seamless care coordination
- Document patient progress and communicate with healthcare teams as needed
- Graduate of an accredited school of nursing
- Active unrestricted license to practice professional nursing in the state of Texas
- Current BLS/CPR certification
- 2+ years of clinical experience in a community home health or medical/surgical setting
- Ability to lift and transfer patients
- Access to reliable transportation
