Job SummaryWe are seeking an experienced Geriatric Physical Therapist to join our team at a Skilled Nursing Facility (SNF). The ideal candidate will possess strong clinical reasoning, communication, and interpersonal skills, with the ability to work effectively as part of a multidisciplinary team.
Responsibilities- Patient Evaluation and Assessment: Conduct comprehensive evaluations of patients' physical and functional abilities upon admission and throughout their care, developing individualized treatment plans based on assessment findings, patient goals, and physician recommendations.
- Treatment Implementation: Provide evidence-based therapeutic interventions, including exercises, mobility training, and pain management techniques, utilizing specialized equipment and techniques to improve strength, balance, coordination, and overall mobility, while monitoring patient progress and adjusting treatment plans as needed.
- Collaboration and Communication: Work closely with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to ensure coordinated patient care, communicating regularly with patients and their families regarding progress, goals, and discharge planning.
- Documentation and Compliance: Maintain accurate and timely documentation in accordance with facility policies, state, and federal regulations, ensuring compliance with Medicare, Medicaid, and other third-party payer requirements.
- Education and Advocacy: Educate patients and their families on exercises, safety precautions, and strategies to maintain progress post-discharge, advocating for patients to achieve their highest level of independence.
