About the Role:

Job Summary

As a vital member of the Interdisciplinary Team, you will be responsible for various functions related to patient admissions, providing service information, and acting as a liaison with both inpatient and outpatient facilities. Your role will involve ensuring a seamless transition of care and effective discharge planning from hospital settings. After patient admission, you will facilitate a smooth handover to the primary nurse and other team members.

Minimum Job Qualifications

  • Licensure or other certifications: Valid Georgia RN licensure. Current BLS certification.
  • Educational Requirements: Associate's Degree. Graduate from an accredited professional nursing program.
  • Minimum Experience: At least one (1) year of clinical nursing experience, particularly in adult care within acute medical and surgical settings or home care preferred.

  • Preferred Licensure or other certifications: CHPN.

  • Preferred Experience: Experience with pediatric and infant care. One (1) year of hospice experience.

Essential Tasks And Responsibilities

  • Evaluate the suitability of patient admissions to the hospice program based on regulatory requirements and hospice admission criteria, in collaboration with the attending physician and hospice Medical Director. Conduct the admission visit using the agency's specialized format within the designated timeframe. Perform comprehensive assessments and collaborate with relevant parties.
  • Initiate the development of a care plan with input from the patient, family, hospice interdisciplinary team, medical director, and attending physician, adhering to agency and regulatory standards.
  • Provide patients, families, and caregivers with detailed information regarding the hospice program, services offered, financial responsibilities, Advance Directives, and the complaint process in compliance with regulations.
  • Implement, assess, and modify nursing and medical care plans in coordination with the physician and interdisciplinary team, following established nursing practice standards.
  • Document patient interactions according to agency policy, utilizing the nursing process and addressing reimbursement issues. Ensure patient charting is completed within 24 hours for all visits.
  • Confirm primary payer status upon admission and ensure proper reimbursement arrangements are established. Communicate with payors as necessary and complete required documentation to facilitate accurate reimbursement.
  • Engage in agency meetings, including interdisciplinary team meetings, staff meetings, and discipline-specific meetings.
  • Coordinate with team members to ensure comprehensive care for each patient admitted to the program, addressing any immediate concerns.
  • Consult with the Medical Director and agency management regarding complex patient situations requiring further evaluation before admission.
  • Manage the patient and family care plan and associated services from admission until transition to the primary nurse.
  • Act as a liaison with physicians, hospital staff, and community agencies to enhance communication and problem-solving between referral sources and the hospice program.
  • Provide timely feedback to hospice management regarding healthcare dynamics and issues affecting the agency's service area.
  • Meet productivity expectations, averaging two complete admissions per day, three initial nursing assessments per day, and/or four ongoing nursing visits per day.
  • Assist primary nurses with their caseloads, cover the Intake department, and support Utilization Review, Quality Assurance, and Performance Improvement activities as assigned.
  • Participate in 24-hour on-call assignments as scheduled.
  • Perform additional duties as required.
