About the Position

This is a PRN/Part Time opportunity for a Nurse Practitioner or Physician Assistant to conduct home risk assessments, reviewing and observing a member's current health, medical history, medication adherence, social environment, and other risks.

The Comprehensive Health Assessment provides valuable information to inform patient care decisions and help achieve improved health outcomes.

Responsibilities

As a Matrix Provider, you will:

  • Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up support.
  • Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.

Requirements

To be successful in this role, you must have:

  • A Master's Degree or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.
  • A valid BLS, ACLS, or CPR certification.
  • A minimum of 1 year experience as a Nurse Practitioner/Physician Assistant, with the ability to travel in-state or out-of-state.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • A positive and outgoing personality, with a commitment to teamwork and adaptability.

Compensation

Earn a competitive salary, ranging from $2,300 to $11,500 per month, based on an average 20-100 completed visits per month. Pay is $115 per completed assessment, with compensation adjusted according to availability and assessment completion.

Benefits

PRN/Part Time providers are eligible for benefits, including a 401(k) plan and limited voluntary benefits. Additionally, you will receive mileage reimbursement and flexible scheduling opportunities.

Travel Requirements

You will need to possess a valid state driver's license and adequate automobile insurance coverage, with the ability to travel to meet patients where they live.
