As a Hospice RN / RN Case Manager, your responsibilities will include:

Reporting directly to the Administrator, Executive Director, or Patient Care Manager, you will oversee the professional management of each patient.

Serving as the primary contact between the physician, patient/family, and hospice team.

Delivering direct patient care services to hospice patients while providing supportive care to their families, maintaining a manageable caseload, and fostering a trusting and compassionate relationship with patients and their families.

Empowering caregivers by educating them on how to care for their loved ones.

Collaborating with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort, peace, and dignity for patients.

Assessing patient and family needs and assisting in the development and implementation of personalized care plans.

Maintaining all assigned patient clinical documentation.

Participating in company quality assessment and performance improvement initiatives.

About You

Requirements for Hospice Registered Nurse / RN Case Manager:

Compassionate Nature:
You should be kind, empathetic, and understanding towards patients and their families.

Comfortable providing care in various settings as designated by the patient.

Ability to demonstrate flexibility and composure while supporting others during challenging times.

RN license in the state of employment (or eligibility to obtain).

A minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology is strongly preferred.

Additional preferred experience that can contribute to your success in a hospice role includes: hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing.

Current driver's license and automobile insurance.

Current CPR certification.
