LPN - Home Health Care Professional

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Bishop's Common at St Luke. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver exceptional outcomes.
Responsibilities- Provide direct patient care according to the therapeutic plan under the supervision of the Nursing Field Supervisor/Director of Patient Services.
- Assess patient needs and develop individualized care plans to meet those needs.
- Administer medications and perform selected treatments as prescribed by the physician.
- Document patient progress and communicate effectively with the healthcare team.
- Collaborate with the RN to prepare and maintain an effective plan of care.
- Reinforce instruction given to the patient/family by the RN or physician.
- Notify the RN of significant findings or changes in the patient's physical and/or mental condition.
- Graduate from an accredited School of Practical Nursing.
- Current valid New York State license and registration as a Practical Nurse.
- Minimum of one (1) year medical/surgical experience, preferred.
- Ability to comprehend and communicate information both verbally and in writing.
- Ability to establish and maintain effective interpersonal relationships.
- Ability to set priorities and organize work.
- Ability to relate to a diverse and multi-cultural population.
- Ability to be flexible and work with minimal supervision.
