Job Summary:

Hospice of the West is seeking a skilled and compassionate Spiritual Care Provider to join our team. As a Per Diem Chaplain, you will provide spiritual care and support to patients and their families in the Tucson area.

About the Role:

As a member of our Interdisciplinary Team, you will be responsible for assessing the spiritual needs of patients and families, providing direct spiritual care, and maintaining accurate documentation of clinical records.

Responsibilities:

  • Assess the spiritual status of patients and families/caregivers related to the patient's terminal illness and environment.
  • Communicate these findings to the RN Case Manager and interdisciplinary group.
  • Provide direct spiritual care for all faiths/non-faiths to patients and families/caregivers in a holistic manner as requested.
  • Maintain regular communication with the Program Director to review clients and help with managing spiritual care needs.
  • Initiate communication with hospice staff members and other facilities as needed to coordinate optimal care and use of resources for patient/family members.
  • Monitor and maintain documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness via electronic record.

Qualifications:

  • Graduate of accredited seminary or school of theology, or appropriate certification in pastoral ministry.
  • Minimum two units of Clinical Pastoral Education (four units strongly preferred).
  • Minimum two (2) years experience as a healthcare pastor/chaplain.
  • One (1) year of hospice experience preferred but not required.
  • Must have ability to handle stress associated with ministering to patients/residents and their loved ones who experience pain, suffering, death, and grief.
  • Must be computer proficient in typing and various programs, including background in EMR.
  • Possess and maintains current CPR certification if required by state.
