COMPENSATION:

KEY RESPONSIBILITIES
As a Care Coordinator, your role will include:

  • Collaborating within a team to achieve the office's objectives and workload
  • Establishing and nurturing trusted, client-focused relationships to ensure client engagement in care services as per the Care Program
  • Acting as a resource for technical inquiries, applications, and problem resolution
  • Assessing, planning, coordinating, and managing Home Care Aides to safeguard the interests of both clients and the organization
  • Maintaining a positive relationship with the Community Care Unit
  • Ensuring that client and employee documentation is thorough and up-to-date

REQUIRED SKILLS:

  • Strong reading and writing abilities
  • Exceptional interpersonal skills
  • Ability to organize and multitask effectively under pressure
  • Basic proficiency in computer and office applications
  • Capability to delegate tasks efficiently

QUALIFICATIONS:

  • Proficient communication skills in English
  • Knowledge of a second language is advantageous
  • High School diploma or equivalent
