Chronic Dialysis Care Manager

**Job Description:
The Chronic Dialysis Care Manager is responsible for providing day-to-day direction and supervision to assigned direct patient care staff, ensuring the safe and effective delivery of patient care in compliance with company, state, and federal regulations. This role requires strong leadership and communication skills, as well as the ability to work effectively in a fast-paced environment.
**Key Responsibilities:
- Leadership and Supervision:
- Provide guidance and support to direct patient care staff.
- Conduct regular performance evaluations and provide constructive feedback.
- Patient Care:
- Develop and implement individualized patient care plans.
- Collaborate with multidisciplinary teams to ensure seamless patient care.
**Requirements:
- Graduate of an accredited school of Nursing (R.N.).
- Current appropriate state licensure.
- Minimum of 9 months of nursing experience and an additional 3 months of clinical experience in dialysis.
**Salary Range:** $75,000 - $95,000 per year.
