We are looking for a seasoned nurse manager to oversee the day-to-day activities of our specialized programs and products. The ideal candidate will have a strong background in nursing, leadership, and program management.

Key Responsibilities:
  • Manage the day-to-day activities of specialized programs and/or products.
  • Establish workflows and protocols, assess their effectiveness, and make recommendations for improvements.
  • Act as a liaison for care management teams to ensure the program is meeting expected outcomes.
  • Initiate, lead, and/or participate in internal and external clinical care conferences.
  • Maintain excellent communication and relationships with home care/hospice teams and Health Plans.
Requirements:
  • License and current registration to practice as a Registered Professional Nurse in NYS.
  • Minimum of three years of clinical experience.
  • Experience in case management, administration, or discharge planning in a hospital setting.
  • Training in population care coordination.
