Position Overview:
Facilitate and deliver essential services and resources to individuals and families aimed at fostering rehabilitation, recovery, and resilience.

Recognize and leverage the strengths that will assist individuals or families in their recovery journey while addressing potential barriers that may hinder the development of skills necessary for independent community living.

Provide support to individuals and families during crisis situations; implement tailored interventions to enhance the individual’s capacity to make informed and autonomous decisions.

The patient care responsibilities may encompass children at risk of or experiencing significant emotional, neurobiological, and/or behavioral disorders; adults with severe mental health conditions, individuals with chronic substance use issues, and those with co-occurring disorders.

Ensure compliance with institutional and departmental policies and procedures. Patient care responsibilities may involve neonate, pediatric, adolescent, adult, and geriatric populations.

Key Responsibilities:

  • PATIENT-CENTERED CARE - Uphold and advocate for the core principles of Patient-Centered Medical Home or Patient-Centered Specialty Practice as applicable.
  • ACCESSIBILITY/ PATIENT ADVOCACY - Strive to remove barriers to culturally competent medical services for all patients. Assist patients in accessing community resources and financial assistance programs.
  • COLLABORATIVE EFFORTS - Partner with medical professionals, multidisciplinary team members, and community resource providers to support patients in achieving their treatment objectives.
  • LIFE SKILLS DEVELOPMENT - Encourage and facilitate the acquisition of skills in areas such as daily living activities, interpersonal coping, socialization, and community engagement. Depending on individual needs, this may involve aiding adaptation to home, school, work, recreational, and social settings; promoting self-care, nutrition, and financial management.
  • PROFESSIONAL GROWTH - Strategize for professional development related to personal goals through self-assessment, evaluation, and feedback; take initiative in acquiring knowledge and experiences to fulfill these goals.
  • WELLNESS PROMOTION - Provide information and resources to educate patients about their specific mental health conditions; serve as a mentor to inspire recovery and resilience, instilling hope; teach skills for symptom monitoring, management, and relapse prevention.
  • MEDICATION MANAGEMENT - Educate individuals about their medications, including side effects, and discuss any concerns with healthcare providers; facilitate self-motivation regarding medication adherence and its implications for independent living.
  • SERVICE PLANNING - Aid in the formulation and coordination of the individual’s service plan based on identified strengths and objectives. This plan will encompass a recovery/resilience management strategy, crisis management plan, and, if requested, advanced directives regarding behavioral healthcare.
  • RESOURCE COORDINATION - Assist clients in accessing and coordinating necessary rehabilitative, medical, and other services, including support in obtaining financial and medical benefits; help secure safe, affordable, and stable housing, and provide support and mentoring to behavioral health consumers involved in the judicial system.
  • SUPPORT SYSTEMS - Foster the development and eventual independence of natural support systems in workplace, school, family, and community settings.
  • ASSESSMENT - Collaborate with individuals and families to identify personal strengths, needs, and obstacles to achieving self-identified goals; conduct ongoing evaluations to ensure that accessed services are effectively meeting the individual’s needs.
  • CRISIS INTERVENTION - Evaluate, support, and intervene during crisis situations, including facilitating the development and utilization of individual crisis management plans that recognize early signs of crisis or relapse, and encourage the use of natural supports. Identify and promote alternatives to emergency department visits and inpatient services.
  • SERVICE TRACKING - Coordinate and monitor service utilization, including comprehensive tracking of client activities in relation to care plans, such as attendance at scheduled appointments, reviewing documentation from other providers, and maintaining communication with external service providers. Conduct follow-up contact post-discharge as appropriate.
  • DOCUMENTATION - Record all client interactions and those conducted on behalf of clients; complete and submit billing documentation as necessary; maintain current and comprehensive client files, which may include documents held for safekeeping on behalf of clients.
  • DATA REPORTING - Maintain and report relevant statistics concerning programs and client services.
  • PROFESSIONAL DEVELOPMENT - Enhance professional growth through participation in educational programs, current literature review, and attendance at workshops and meetings.
  • PATIENT SAFETY - Adhere to patient safety-related policies, procedures, and protocols.
  • PATIENT SAFETY - Demonstrate a proactive approach to patient safety by seeking opportunities to improve safety through questioning existing policies and processes.
  • PATIENT SAFETY - Identify and report/correct environmental conditions or situations that may pose undue risk to patients.
  • PATIENT SAFETY - Promptly report potential or actual patient safety concerns, medical errors, and near misses.
  • PATIENT SAFETY - Encourage patients to actively engage in their own care by asking questions and reporting any treatment or situations that are unclear or concerning.

  • UNM Vehicle Operator's Permit within 60 days

  • Licensed Alcohol and Drug Abuse Counselor or LASI eligible

Physical Requirements:

Light Work:

Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. Physical demand requirements exceed those for Sedentary Work.

May require significant walking or standing or entails sitting most of the time but involves pushing and/or pulling of arm or leg controls; may require working at a production rate pace involving constant pushing and/or pulling of materials even if the weight is negligible.
