  • Track and manage appointment cancellations
  • Discharge participants after clinic visits
  • Prepare packets for specialty appointments
  • Provide daily schedules to primary care teams
  • Attend participant care team meetings and address identified issues

Requirements:

  • Ability to work in a fast-paced environment
