We are seeking a highly skilled and experienced Registered Nurse to join our Cardiac Rehabilitation team at CentraState Healthcare System. As a Cardiac Rehabilitation Nurse Specialist, you will play a vital role in providing high-quality patient care and contributing to the success of our team.
Key Responsibilities- Assess and Plan Care: Conduct thorough assessments of patients to identify their unique needs and develop individualized care plans that promote optimal health outcomes.
- Implement and Evaluate Care: Implement care plans and evaluate their effectiveness, making adjustments as needed to ensure the best possible outcomes for patients.
- Collaborate with Interdisciplinary Teams: Work closely with physicians, therapists, and other healthcare professionals to ensure seamless care coordination and communication.
- Provide Education and Support: Educate patients and their families on cardiac rehabilitation, healthy lifestyle choices, and disease management to empower them to take control of their health.
- Manage and Coordinate Care: Oversee the daily operations of the cardiac rehabilitation program, including managing patient assignments, coordinating with other departments, and ensuring compliance with regulatory standards.
- Education: Bachelor of Science in Nursing (BSN) degree required.
- Experience: Two years of experience in telemetry or critical care required; experience in emergency care considered an asset.
- Licensure: RN License in New Jersey required.
- Certifications: BLS and ACLS certifications required; specialty certification in cardiac rehab required or obtain within two years of employment.
