Nurse Case Manager I

The Nurse Case Manager I plays a vital role in ensuring the optimal care and well-being of patients at Medical University of South Carolina (MUSC). As a member of the clinical team, this position coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge.
Key Responsibilities- Assess, plan, implement, monitor, and evaluate the options and services required to meet an individual's health needs.
- Coordinate and manage the care of patients to meet multiple service needs across the continuum of care.
- Partner with the multidisciplinary team to establish care and allocate resources associated with the patient's risk assessment.
- Assist the patient/patient's family in coping with illness by optimizing the patient's/family's self-care abilities and supporting their consumer rights.
- Bachelor's degree in Nursing from an accredited school of nursing.
- Three years work experience as a Registered Nurse required.
- Prior leadership experience preferred.
- One year of case management and/or utilization review work experience preferred.
- Case Management certification preferred.
The Nurse Case Manager I will work in a fast-paced environment, requiring strong communication and organizational skills. The ability to work rotating shifts and overtime as required is essential.
