ChristianaCare is seeking a dedicated and compassionate Registered Nurse to join our Family Medicine Center in Wilmington, DE.
About the Role:We are looking for a skilled RN to provide high-quality patient care in a fast-paced family medicine setting. As a member of our team, you will work closely with physicians, advanced practice clinicians, and other healthcare professionals to deliver exceptional patient outcomes.
Responsibilities:- Perform Annual Wellness Visits, Advance Care Planning, and Transitional Care Management calls and disease management.
- Triage walk-in patients and ensure appropriate disposition.
- Telephone triage and answer/respond to telephone messages under the direction of the provider.
- Assist providers in examinations or procedures.
- Function as a liaison between patients, physicians, and staff to provide accurate communication.
- Assure adequate coverage within the unit when needed.
- Support the work of LPN and/or MA.
- Assist with staff education and evaluation of clinical competencies for new and existing employees (MA & LPN).
- Assist with orientation of new personnel to ensure they are properly trained to support patient and provider.
- Perform chart audits or have a designee assist with chart audits.
- Case Management and interdisciplinary collaboration.
- Chronic disease management under provider direction.
- Perform nurse appointments for patient education and medication/vaccine administration.
- In-office medication administration and wound care.
- Full spectrum family medicine practice, including prenatal care.
- Support Substance Use Disorder Programs in Family Medicine office and Mobile Van, including coordination of care and patient outreach.
- A graduate of a state-approved or National League of Nursing accredited School of Nursing.
- RN license or eligible for licensure in the state of Delaware.
- Two years' experience as a RN, physician office experience preferred.
- BSN preferred; ADN considered with agreement of completion of BSN in 3 years.
