Job Summary

The Transplant Coordinator RN plays a critical role in our healthcare team, performing patient care coordination tasks and ensuring seamless care for transplant patients.

Responsibilities include assessing patients, developing personalized care plans, and monitoring patient responses.

The ideal candidate will have a deep understanding of nursing and patient care standards, as well as excellent communication and critical thinking skills.

Key Responsibilities
  • Assess transplant candidates and develop personalized care plans
  • Coordinate testing and care for waitlist candidates
  • Monitor test results and communicate with patients and healthcare teams
  • Manage aftercare for transplant recipients
  • Partner with physicians to monitor complications and abnormal test results
