We are seeking a highly motivated and dedicated Case Manager - Social Services Professional to join our team at Metropolitan Educational Solutions. As a key member of our social services department, you will work closely with clients, families, and community stakeholders to provide comprehensive support services.
Key Responsibilities:
- Develop and implement individualized service plans for clients.
- Provide case management services, including assessment, goal-setting, and resource coordination.
- Collaborate with community partners to ensure seamless delivery of services.
- Maintain accurate records and reports on client progress.
- Participate in staff meetings and training sessions to enhance skills and knowledge.
Requirements:
- Bachelor's degree in social work, psychology, sociology, or related field.
- Minimum 2 years' experience working with at-risk youth or vulnerable populations.
- Certification in a relevant discipline (e.g., certified addictions counselor) may be required.
- Excellent communication, interpersonal, and organizational skills.
