Job Title: Full Time Hospice RN Case Manager

We are seeking a skilled and compassionate Registered Nurse to join our team as a Full Time Hospice RN Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in need of hospice services.

Job Summary:

The Full Time Hospice RN Case Manager will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating with healthcare providers to ensure seamless care delivery. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team
  • Coordinate with healthcare providers to ensure timely and effective care delivery
  • Provide education and support to patients and their families on end-of-life care and related topics
  • Collaborate with the hospice team to ensure comprehensive care and support
  • Document patient information and care plans accurately and thoroughly
Requirements:
  • Current RN licensure in the State of Georgia
  • At least one year of experience in hospice care or a related field
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Proficiency in electronic medical records and other relevant software
