The primary program we offer, known as The Bridge, focuses on supporting youth and young adults aged 14 to 29. Our goal is to empower them to achieve self-sufficiency and meet their aspirations across five key life transition areas: Employment & Career, Living Situation, Educational Opportunities, Community Life Functioning, and Personal Effectiveness & Wellbeing. Our approach is collaborative, involving a dedicated team that includes a Youth Transition Specialist, Personal Wellness Coach, and Peer Support Specialist to guide clients on their journey.
Position SummaryThe Youth Transition Specialist plays a crucial role in working with youth and their families to enhance functional behaviors and help them overcome mental health challenges. This position provides direct support and coaching in the five transition areas, offering a range of rehabilitative and case management services. Responsibilities include prevention planning, vocational training, skill development, and connecting clients to community resources and educational opportunities.
QualificationsEducation:
• A Master's degree in social work, child counseling, psychology, social services, vocational counseling, or a related field, OR a Bachelor's degree in a similar discipline.
Experience:
• Prior experience working with youth and young adults who have been involved in the foster care system is preferred.
• Strong ability to connect with youth and families from diverse backgrounds.
• Capacity to deliver culturally sensitive services that respect the unique characteristics of the population served.
• Proficient in verbal and written communication with various stakeholders, including youth, families, and community partners.
• Valid driver's license and reliable transportation for client visits.
• Competence in documentation and case management.
• Ability to identify and address barriers to client functioning and implement appropriate interventions.
• Familiarity with community resources and the ability to establish connections for clients.
• Build trusting relationships with clients to foster hope and empowerment.
• Conduct assessments to identify strengths and needs, collaborating with clients to create actionable Future Plans.
• Provide in-vivo coaching, teaching skills in real-world settings, and recognizing opportunities for learning.
• Assist clients in managing high-risk behaviors and developing decision-making skills.
• Deliver trauma-informed care and recognize the effects of trauma on clients.
• Identify and refer clients to community-based resources, ensuring they receive the necessary support.
• Maintain accurate records and track outcomes related to transition areas.
• A valid driver's license and proof of insurance are required.
• Commitment to professional standards and adherence to ethical guidelines.
• Participation in ongoing training and professional development is expected.
The New Foster Care is a non-profit organization based in Oakland County, established to support young individuals with foster care experience in transforming their lives. Our mission is to advocate for innovative programming and policy reform that positively impacts the foster care system. Through our comprehensive Bridge Program, we provide tailored support to youth aged 14 to 29, helping them build sustainable plans for their future across essential life domains.
