We are seeking a skilled and compassionate Transitional Aide to join our team at Civility Home Care. As a Transitional Aide, you will play a vital role in facilitating the transition of clients to their home and ensuring they receive the necessary support to maintain their independence.
Key Responsibilities:- Provide personal care services to clients in their own homes and communities, including assistance with activities of daily living, housekeeping, and meal preparation.
- Ensure client safety and security by supervising the home environment and reporting any concerns to the Supervisor.
- Teach and perform meal planning and preparation, routine housekeeping activities, and provide companionship and social interactions.
- Perform respite care for families in accordance with care plans and escort clients to medical facilities, errands, and outings as specified.
- Assist clients with communication, including writing or typing correspondence and researching information.
- Participate on the Care Team by providing input and making suggestions to improve the program.
- Certification in Personal Care
- Current driver's license and proper vehicle insurance coverage
- Related experience and on-the-job training for new activities
- Ability to work independently and in cooperation with others
- Good physical and mental health, with physical ability to stand, walk, and use hands and fingers
Civility Home Care offers a flexible schedule, including overnights and day shifts, as well as on-call opportunities. We provide on-the-job training and opportunities for professional growth and development.
