The Orthopedic Nurse Coordinator for Spine Care plays a vital role within the interdisciplinary Spine team. This position is dedicated to facilitating comprehensive care for spine patients, aiming for outstanding patient experiences and clinical outcomes.
The Nurse Coordinator enhances patient readiness for treatment through education, assessment of clinical needs, and support, while also fostering communication between patients, their healthcare providers, and necessary referrals.
Working closely with Spine leadership and physician teams, the Nurse Coordinator ensures that patients receive holistic care while optimizing access and efficiency.

- Patient/Family Assessment
1.1 Gathers comprehensive patient information for new patients prior to their first appointment using telecommunication methods. This includes physical and psychological history, current health status, medication review, and educational needs assessment.
1.2 Reviews data collected by scheduling or intake personnel to confirm completeness and identify any gaps, ensuring all necessary data and imaging are available before the initial visit.
1.3 Monitors patient/family responses to medical, nursing, and therapy interventions at regular intervals (following the initial visit and nearing the completion of the treatment plan).
1.4 Communicates assessment findings to relevant team members and documents them in the electronic medical record (EMR) in accordance with policy. - Patient/Family Education
2.1 Develops a tailored patient education strategy based on assessment results, covering preoperative and/or postoperative care, disease understanding, medications, treatments, and care plans.
2.2 Instructs patients on effective communication with the healthcare team.
2.3 Provides written materials via handouts and/or email to reinforce verbal instructions.
2.4 Assesses patient/family comprehension of the information provided and documents findings in the EMR.
2.5 Supports patients and families throughout their experience to ensure adherence to the treatment plan. - Patient/Family Referrals
3.1 Coordinates and initiates referrals to physical and occupational therapy, case management, and community resources as necessary.
3.2 Collaborates with physicians, advanced practice providers, and schedulers to ensure appropriate scheduling of studies, procedures, and interventions following the initial visit, serving as a crucial link between patients and other care providers.
3.3 Anticipates and identifies potential barriers to patient adherence to the treatment plan and works with other providers/team members to mitigate these challenges. - Spine Program Support
4.1 Establishes and maintains collaborative relationships with physicians, advanced practice providers, nurses, technologists, schedulers, practice managers, front desk staff, physical therapists, and ancillary support roles as the Spine program evolves.
4.2 Collaborates with advanced practice providers to triage complex inquiries.
4.3 Participates in the development and implementation of evidence-based clinical tools relevant to new patient intake, access, and navigation.
4.4 Identifies and monitors program quality metrics pertinent to the patient population, including outcomes and trends.
4.5 Engages in and leads program-specific safety and quality improvement initiatives. - Professional Development
5.1 Maintains clinical expertise through participation in grand rounds, relevant conferences, continuing education, professional organizations, and other related activities.
5.2 Under the guidance of Spine leadership, engages in specific quality improvement activities relevant to the program and role.
5.3 Fosters and exemplifies a learning environment that promotes patient safety and evidence-based practices for all levels of patient care staff.
Qualifications:
EDUCATION
Bachelor's degree in nursing is required; a Master's degree in a health-related field is preferred.
EXPERIENCE
3-5 years of experience with surgical patient populations, either inpatient or outpatient. Experience in neurosurgery, spine, and/or orthopedic care is preferred.
LICENSURE
Connecticut RN Nursing license, BLS certification, and specialty certification in Neurosciences or Orthopedic nursing are preferred.
PHYSICAL DEMAND
Must be capable of assisting patients on and off procedure tables and pushing wheelchairs.
Additional Information:
BLS certified, with two years of experience in orthopedic/neurosurgery. Previous experience in case management or coordination is preferred but not mandatory.
