Home Care Support Specialist

Position Summary
The Family Home Provider is dedicated to ensuring a safe, nurturing, and therapeutic atmosphere for individuals in care, while diligently implementing their personalized care plans.
This role may involve offering direct assistance and guidance in various aspects of daily living, including but not limited to: communication enhancement, home management, personal hygiene, behavioral support, nutritional planning, employment assistance, financial management, educational engagement, and recreational activities.
Key Responsibilities- Safeguard the health and well-being of individuals in care.
- Administer medications and treatment plans as required.
- Support individuals in maintaining a clean and organized living space, including reporting maintenance issues.
- Assist with financial management, including budgeting, banking, and shopping.
- Facilitate daily living activities such as meal preparation, communication, transportation, and personal care.
- Make necessary purchases for household needs.
- Prepare nutritious meals.
- Provide crisis intervention when necessary.
- Complete all required documentation in compliance with state regulations and organizational policies.
- Implement behavioral programs as trained and requested.
- Support individuals in achieving their personal goals as outlined in their care plans.
- Identify and manage potential behavioral triggers to ensure safety and well-being.
- Be available for support 24/7.
- Provide transportation for individuals as needed.
- Manage health-related tasks, including scheduling and attending medical appointments, monitoring medications, and following up with healthcare professionals.
- Ensure that the living environment is safe, clean, and well-maintained.
- Monitor and manage petty cash and budgetary requirements for individuals.
- Must be at least 21 years of age.
- Proficient in reading, writing, and verbal communication in English.
- Meet all state agency requirements, including maintaining good standing with abuse and neglect registries.
- Basic computer skills are necessary.
- Prior experience in direct support roles is preferred.
- Ability to move about to coordinate tasks.
- Occasional physical assistance to individuals as needed.
- Remain alert to the needs of individuals.
- Occasional exposure to health risks.
