About the Role

We are seeking an experienced and skilled Nurse Practitioner to join our team in Montgomery County, PA. This is a fantastic opportunity to provide high-quality care to patients in their homes, working collaboratively with our clinical operations team.

Key Responsibilities:

  • Provide primary care services to homebound patients in Montgomery and Bucks counties, PA.
  • Assess patient health through physical examinations, medical histories, and diagnostic testing.
  • Develop effective treatment plans and prescribe medications as necessary.
  • Foster trust and communicate effectively with patients, families, and caregivers during visits.
  • Collaborate with our in-house clinical team to ensure seamless care delivery.
About the Position

This is a full-time position offering a competitive salary range of $105,000 - $145,000+, depending on experience and qualifications.
