Overview:
Camilla Hall, a part of Trinity Health PACE, provides high-quality care to seniors in the communities we serve. Our interdisciplinary team offers comprehensive services, allowing seniors to remain independent at home.
We are seeking an experienced Primary Care Physician to join our team. As a Primary Care Physician, you will provide primary medical care to PACE Organization participants, including assessment, development of a plan of care in collaboration with the interdisciplinary team, provision of direct participant medical care, and evaluation of effectiveness of the plan of care.
The successful candidate will work in Camilla Hall as well as completing nursing home/other religious community visits outside of Camilla Hall. The position requires approximately 10th week on-call rotation. Typically, regular schedule does not require night, weekend or holiday shifts. However, may work these times when on call.
Responsibilities:
- Participate as a member of the interdisciplinary team (IDT) and in initial, semi-annual, unscheduled, and annual assessments.
- Provide initial and ongoing participant assessments, care plan development, and implementation of care plan.
- Refer participants to medical specialist as indicated. Manage all care of participants in the nursing home: Provides regular visits to manage the care of participants in the nursing home in collaboration with the Interdisciplinary Team; maintains communication with nursing home staff for continuous monitoring of PACE participants on a case-by-case basis; admits nursing home participants to hospital when necessary.
- Oversees and manages participants use of medical specialist and inpatient care. Communicates and follows up with specialist regarding outcome of visit.
- Review medical outcomes to establish best practices and improve participant health. Complete documentation and uses appropriate diagnostic coding.
- Advise the Medical Director in ways and means to establish better accountability of PO services to participants and referral sources, keeps Medical Director aware of needed material and human resources as program expands.
- Assist with the development of policies and procedure, standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.
- Participate in provider on-call rotation as required.
Requirements:
- Doctorate Degree: MD or DO. Current and active plenary physician license to practice in state of PO required. CDS Certification and DEA Registration required.
- Current BLS required.
- Board Certification Internal Medicine or Family Practice required. Board Certification in Geriatrics preferred.
- A minimum of one (1) year working with the frail or elderly population required
- Must have a valid drivers license, proof of insurance and use of personal vehicle required.
- Strong communication, problem-solving, and organizational skills. Able to communicate effectively with individuals and groups representing diverse perspectives.
- Proficiency with Microsoft Office suite
- Ability to change and adapt to changing priorities.
- Must exhibit critical thinking skills and possess the ability to prioritize workload.
- Periodic travel within the Center's geographic region.
