At AbsoluteCare, we're seeking a highly skilled Primary Care Clinical Coordinator to join our team. As a key member of our clinical staff, you'll play a critical role in providing high-quality care to our patients.

About the Role

This is a full-time position that offers a competitive salary of $80,000 - $110,000 per year, depending on experience. Our ideal candidate will have a strong background in nursing and excellent communication skills.

The Primary Care Clinical Coordinator will be responsible for:

  • Providing community-based medical and care-coordination services for recent hospital discharges;
  • Managing a small panel of homebound members who require primary care;
  • Resolving acute issues in the home and clinic or by directing members to the most effective and efficient solutions;
  • Providing on-site and home visit clinical care, including evaluation and treatment of acute illnesses, chronic conditions, and infections;
  • Ensuring coordination of care, including timely and accurate completion of chart documents and collaboration with the Treatment and Triage Nurse;
  • Communicating important post-visit follow-up information to the Core Clinical Team;
  • Collaborating with the Integrated Care Team (ICT) members for problem-solving and coordination of care;
  • Identifying and referring members to specialty programs when instability or crisis occurs;
  • Implementing transitional care protocols as part of warm hand-offs between internal and external care providers;
  • Helping patients identify barriers to meeting health and life goals and connecting them with resources to navigate these barriers;
  • Adjusting care plans following guidelines for evidenced-based chronic disease management;
  • Educating and coaching patients on medication adherence and chronic disease self-management skill development;
  • Moderating members at risk of being lost to care and implementing interventions to prevent loss.
