Job Summary

The Health Coordinator role at Pioneer Human Services is a unique opportunity to make a meaningful impact in the lives of our clients. As a key member of our team, you will be responsible for coordinating access to quality and cost-effective integrated health services. This involves working closely with internal staff, community providers, and facility staff to ensure that our clients receive the best possible care.

Key Responsibilities
  • Coordinate access to integrated health services for clients at Pioneer facilities
  • Work with internal staff, community providers, and facility staff to gather and relay necessary information
  • Assist in assessing and managing client integrated health needs

Requirements
  • Valid Driver's License
  • 1-2 years of experience working with marginalized populations
  • High School Diploma or Equivalent
  • Strong written and verbal communication skills, customer service skills, and attention to detail
  • Proficiency in Microsoft Office Suite, Outlook, and Internet navigation
  • Knowledge of electronic records management and confidentiality
