Your Role:

The Ambulatory Care Pharmacist plays a vital role in supporting the mission of the department, hospital, and health system by identifying, preventing, and resolving medication-related issues to ensure patient safety, enhance patient satisfaction, optimize outcomes, and reduce costs. Responsibilities may include working in both centralized and decentralized settings. The role encompasses all functions of a licensed pharmacist, including precise and timely medication dispensing, clinical interactions with patients and healthcare teams, and supervision of pharmacy technicians.

Your Contributions to the Team:

a) Pharmacy Operations – Actively engages in all pharmacist duties, adheres to established operational standards, and maintains proficiency in central and satellite pharmacy services as needed. Ensures effective use of medication automation and technology. Manages medication inventories and oversees the preparation and dispensing of patient-specific medication doses. Supervises pharmacy technicians and interns, prioritizes tasks, organizes workflow, and verifies the accuracy of technician activities.

b) Pharmacy Practice – Conducts thorough medication order reviews, drug profile assessments, and patient monitoring to identify and mitigate medication-related issues. Effectively verifies and enters medication orders while proactively communicating with medical staff to resolve any discrepancies. Provides patient-centered care services, ensuring safe and timely medication therapy, and participates in cost-effective drug therapy initiatives.

c) Clinical Pharmacy Services – Actively engages in direct patient care programs, delivering accurate, safe, and timely medication therapy tailored to patient needs. Collaborates with healthcare providers and pharmacy staff, prioritizing patient-centered care delivery.

d) Teamwork and Inter-professional Collaboration – Arrives on time and prepared to work, ensuring continuity of care across shifts and pharmacy assignments. Collaborates effectively with all healthcare team members to achieve optimal patient outcomes and demonstrates sensitivity to the perspectives of patients and colleagues in all communications.

e) Teaching, Training, and Mentoring – Provides mentorship to new staff and assists in the training of all pharmacy personnel as needed. Supports the experiential training of pharmacy trainees and mentors pharmacy students as required. Demonstrates a commitment to teaching and maintains the ability to deliver effective, evidence-based education to nurses, patients, and pharmacy team members.

f) Quality Improvement and Compliance – Participates in quality improvement initiatives aimed at enhancing medication-related processes and patient care programs. Supports compliance with regulatory standards and institutional accreditation processes.

Qualifications Required for Success:

Licensure, Certification, or Registration:

  • Bachelor of Pharmacy, Doctor of Pharmacy, or equivalent advanced pharmacy degree
  • BLS Certification
  • Valid State Pharmacy License

Preferred Qualifications:

  • PGY2 in Ambulatory Care or Cardiology with outpatient clinic experience
  • PGY1 Pharmacy Residency or three years of clinical pharmacy experience
