Job Description

We are seeking a skilled Patient Care Manager to join our team at Assured Home Health, a part of LHC Group. As a Patient Care Manager, you will play a vital role in the supervision and coordination of clinical services, ensuring high-quality care for our patients in their homes.

Responsibilities
  • Supervise and coordinate clinical services to ensure compliance with applicable laws, regulations, and agency policies.
  • Provide and direct provisions of nursing care to patients in their homes as prescribed by the physician.
  • Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
  • Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
  • Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
Qualifications
  • Current RN licensure in state of practice.
  • Current CPR certification required.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
