About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Health Care and Rehabilitation Services (HCRS). As a key member of our Care Coordination Program, you will play a vital role in providing care coordination, case management, and resource connection services to our clients.

Key Responsibilities
  • Provide care coordination and resource connection services to clients, including referrals to treatment and community resources.
  • Develop and implement time-limited care coordination plans that target clearly identified care coordination goals with a means of measuring success and program completion.
  • Collaborate with community team, other providers, and partnering community agencies to establish relationships and provide support to clients.
  • Complete periodic reports as required by the agency and state, and maintain accurate and timely clinical documentation in the Electronic Medical Record.
Requirements
  • Bachelor's degree in Social Work or related field (or equivalent experience).
  • Excellent communication and interpersonal skills, with the ability to engage, empathize, and have compassion for clients.
  • Knowledge of developmental trauma, attachment issues, mental health, substance use, and behavioral issues that impact an individual's quality of life.
  • Intermediate or advanced computer skills, with experience using electronic medical records.
