About the Role:
The In-Home Assessment Nurse Practitioner (IHA NP) will assess patient/family psychosocial needs, plan, implement, procure services for, and evaluate care needs while meeting with members in their homes. This role requires traveling up to 90% throughout designated regions, conducting in-home assessments, formulating medical conditions, identifying urgent situations, educating members on health topics, providing care coordination, and promoting quality of life.
Key Responsibilities:
- Travel extensively throughout designated areas
- Conduct in-home assessments using established guidelines
- Formulate medical conditions using clinical knowledge
- Educate members on health topics and continuation of care
- Provide care coordination to address behavioral, physical, and social issues
- Foster positive relationships with clients, caregivers, providers, and team members
Requirements:
- Master's Degree in Nursing required
- Current Licensed Nurse Practitioner for Arizona
- Current CPR certification
- Bilingual English/Spanish preferred
