Position Overview:
The Practical Nursing Specialist (LPN) will operate under general supervision to deliver a blend of nursing services, patient education, and assessments across our healthcare facilities. This role involves engaging in various nursing evaluations, providing direct patient care, and facilitating health education initiatives.

The LPN is responsible for ensuring that patient care is delivered in accordance with established standards, policies, and procedures set forth by American Health Partners and relevant regulatory bodies.


Key Responsibilities:
To succeed in this role, the individual must effectively perform the following essential functions:
1. Oversee all patient treatments and care plans.
2. Assist medical professionals in the assessment and treatment of patients.
3. Execute administrative tasks including documentation of nursing notes, physician orders, and discharge plans.
4. Manage medication orders from the pharmacy and administer medications as prescribed.
5. Conduct and monitor prescribed medical treatments and procedures.
6. Perform emergency interventions as necessary and communicate with family members in critical situations.
7. Maintain inventory and requisition necessary medical supplies.
8. Facilitate patient admissions, transfers, and discharges.
Qualifications:
1. A minimum of six (6) months of experience in a healthcare setting, such as a hospital or long-term care facility.
2. Completion of a Licensed Practical Nursing (LPN) program.
3. Current state LPN licensure is required.

Additional Requirements:
This position may require occasional overtime and weekend shifts. Flexibility in scheduling is essential.

Physical Demands:
The role involves frequent lifting of objects weighing up to 50 pounds.
