Caregiver Liaison for Mental Health Support

Our comprehensive approach involves collaborating with clients at various stages of their healing process, utilizing a multidisciplinary team and a multi-tier program of available options.

Job DescriptionThis part-time position, with potential for full-time, requires a unique blend of skills and experience. As a Caregiver Liaison, you will be responsible for:
- Conducting weekly face-to-face sessions with clients, tailored to their specific needs and preferences.
- Maintaining open communication with case managers, reporting any notable interactions or observations.
- Providing daily phone support, home visits, medication adherence assistance, life skills coaching, recovery-related activities, and community development guidance.
You will work closely with clients, family members, outside clinical providers, and support personnel to create and implement personalized reintegration plans, promoting skill building and emotional regulation.
Required Skills and QualificationsTo succeed in this role, you should possess:
- Familiarity with mental health treatment and services, with preferred experience in trauma-informed and addiction-informed practices.
- CPR certification and a valid driver's license with a clean driving record.
- Strong problem-solving skills, ability to manage potential issues, and maintain a flexible schedule, including weekends and holidays.
- Excellent communication and interpersonal skills, with the capacity to remain empathetic and professional in challenging situations.
The estimated hourly rate for this position is between $20.00 and $36.00, depending on qualifications and assignments.
