Long-Term Care Nurse LPN/RN Opportunity

Sunset Rehabilitation and Health Care is hiring compassionate and dependable Licensed Practical Nurses (LPN) and Registered Nurses (RN) to join our growing team.

The Role:

The LPN/RN provides direct patient care under the supervision of the Director of Nursing or physician. Key responsibilities include administering medications, taking vital signs, performing wound care, and assisting patients with daily living activities. Additionally, you will document patient care in the electronic health record system and collaborate with other healthcare professionals as needed.


Responsibilities:
  • Administer medications as prescribed by a physician or Director of Nursing
  • Monitor and record patient vital signs, such as blood pressure and heart rate.
  • Communicate effectively with patients and their families about their care and treatment plan.
  • Document patient visits and treatment plans in the electronic medical records system
  • Collaborate with other healthcare professionals, such as registered nurses, physicians, and therapists.
  • Complete patient assessments, coordination of care
  • Identify and report changes in the resident's condition.
  • Follow infection control and safety protocols to ensure patient and staff safety.
  • Maintain confidentiality and ethical standards in all patient interactions.

Requirements:
  • Valid Illinois Nursing License LPN or RN
  • Experience in long-term care or healthcare setting preferred
  • Excellent communication and interpersonal skills
  • Strong attention to detail and ability to follow instructions accurately
  • Ability to arrive on time for all scheduled shifts
  • Must have the ability to read, write, and follow oral and written directions at a level necessary to accomplish the job
  • Detail-oriented with strong organization and prioritization skills

The estimated salary for this role is $$65,000 - $80,000 per year, depending on experience and qualifications. We offer a comprehensive benefits package, including health insurance, retirement savings plan, and paid time off.
