As a key member of the Nystrom & Associates, Ltd. team, you will play a vital role in providing high-quality care to adult individuals in their substance abuse recovery process at our 54-bed residential facility. Our facility is designed to provide Substance Use Disorder and Co-Occurring services to men and women over 18 years of age.
Responsibilities include:
- Deliver exceptional care by administering and monitoring medications, treatments, and daily cares to ensure the well-being of our patients.
- Complete technical admission and discharge procedures for patients entering and leaving the unit, ensuring seamless transitions and continuity of care.
- Participate as a key member of the multidisciplinary team to develop and implement treatment plans tailored to the unique needs of each individual, promoting optimal recovery outcomes.
- Work under the direction of the Residential Registered Nurse to ensure effective coordination of care and services.
- Respond to patient calls for medication assistance in coordination with the patient's Medication Provider, ensuring timely and accurate support.
- Review, prioritize, and respond to patient and provider messages, tasks, and voicemails within 24 hours, maintaining open communication and collaboration.
- Meet with patients as needed to discuss medication side effects and other concerns, providing education and support to promote patient engagement and empowerment.
- Refill medications as directed by Providers or per policy and procedure, ensuring continuity of care and minimizing disruptions.
- Collaborate with insurance companies as needed for medication authorizations, ensuring timely and accurate processing.
- Request lab results from area clinics and laboratories, maintaining accurate and up-to-date patient records.
- Process incoming medical reports and forms in a timely fashion, ensuring seamless integration into patient records.
- Review and prioritize lab results within 24 hours of receipt, transcribing incoming lab results within 48 hours of receipt, maintaining accurate and up-to-date patient records.
- Perform clinical duties, including but not limited to: rooming patients, taking vital signs, and conducting other assessments as requested by Providers or per policy and procedure, ensuring comprehensive and high-quality care.
