Home Health Care Nurse

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Care Nurse. This role will involve providing personalized care and support to clients in their homes, working closely with them and their families to develop and implement comprehensive care plans that meet their unique needs and goals.

• This is a full-time opportunity with flexible scheduling options available.
• You will be responsible for conducting home visits to assess clients' conditions, providing medical care and interventions as needed, and documenting all interactions and observations.
• As a Home Health Care Nurse, you will work independently and manage your time effectively to ensure timely completion of tasks and delivery of quality care.
• You will also collaborate with other healthcare professionals to coordinate care and address any concerns or issues that may arise.

1. Conduct thorough assessments of clients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
2. Administer medications and treatments as prescribed by physicians, monitor clients' responses, and adjust care plans accordingly.
3. Provide education and guidance to clients and their families on managing chronic conditions, using assistive devices, and maintaining healthy habits.
4. Collaborate with interdisciplinary teams to coordinate care and address complex client needs.
5. Document all interactions, observations, and interventions in electronic medical records, ensuring accurate and timely reporting.

• A current license as a Registered Nurse in PA.
• A minimum of one year of recent, verifiable clinical experience, preferably in home health care or a related field.
• Prior experience with electronic medical records (EMRs) and proficiency in using touch pad tablets preferred.
• Ability to travel to cases as assigned, with reliable transportation and a valid driver's license.
• Strong interpersonal skills, with excellent communication and teamwork abilities.

• A competitive salary range of $65,000 - $85,000 per year, depending on experience.
