Compassionate Home Health Nurse

We are seeking a compassionate and skilled Registered Nurse to join our team as a Home Health Nurse. As a member of our team, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.

• Develop and implement personalized care plans for clients based on their unique needs and goals.
• Conduct thorough assessments of clients' physical and emotional well-being, identifying areas for improvement and developing strategies for addressing these concerns.
• Provide direct care services, including wound care, medication management, and education on disease management and self-care techniques.
• Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless coordination of care and effective communication with clients and families.
• Document all interactions, interventions, and outcomes in accordance with regulatory requirements and company standards.

• A current license as a Registered Nurse in PA.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills and ability to communicate effectively with diverse populations.
