Job Title: Family Nurse Practitioner - Paragon Infusion Centers

We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our team at Paragon Infusion Centers. As a key member of our healthcare team, you will provide exceptional patient care and support to our patients in a fast-paced and dynamic environment.

Job Summary:

The Family Nurse Practitioner - Paragon Infusion Centers is responsible for delivering high-quality patient care, conducting physical assessments, and developing treatment plans in collaboration with our multidisciplinary team. You will work closely with our patients, families, and healthcare providers to ensure seamless care coordination and optimal patient outcomes.

Key Responsibilities:
  • Administer ordered therapies according to prescriber orders, facility protocols, and company policies.
  • Complete admission processes as directed by the Center Director.
  • Perform physical assessments on patients as warranted and for new consults.
  • Document all pertinent data in the patient's medical record.
  • Communicate effectively with other team members on patient status and observations.
  • Maintain patient confidentiality and adhere to HIPAA laws and regulations.
  • Evaluate patient responses to therapy and document findings.
  • Provide education to patients and families regarding therapy plans.
  • Participate in Quality Improvement programs.
  • Ensure accurate and up-to-date medication orders prior to treatment.
  • Participate in On-Call programs, including after-hours, weekend, and holiday calls and visitations.
Requirements:
  • Master's degree in Nursing and a minimum of 2 years of related experience with clinical emphasis for nurse practitioners.
  • Minimum of 1 year experience with IVs required.
  • Current, active, valid, and unrestricted NP license to practice as a healthcare professional within the scope of license in applicable states.
  • Active, current, and valid Family Nurse Practitioner Certification.
  • Multi-state licensure required if providing services in multiple states.
  • Satisfactory completion of a Tuberculosis test and Hepatitis B vaccine or signed waiver.
Preferred Skills and Experiences:
  • Ability to travel to worksite and other locations as necessary.
  • Minimum of 1 year healthcare experience with IVs strongly preferred.
