We are seeking a highly skilled Community Health Specialist to join our team at Carolina Family Health Centers, Inc.

Job Overview:

The Community Health Specialist will provide diagnosis, brief behavioral health intervention, and psychotherapy for patients with mental health and substance use disorders as part of the primary care treatment team.

Key Responsibilities:

  • Educate staff via in-services on related behavioral health topics when needed
  • Teach treatment enhancement techniques using non-stigmatizing language and empathy to patients, families, and staff
  • Assess the clinical status of patients referred by primary care providers and outside referral sources to develop a patient-focused plan of care for substance use disorder and/or mental health treatment
  • Incorporate chronic disease management into overall treatment goals
  • Use evidence-based assessment screening tools with patients to assist with identifying concerns and outcome measurements
  • Consult with medical providers as necessary, and refer cases to specialty, case management, and other behavioral health/substance use providers as appropriate
  • Work with primary care team to treat and manage patients with substance use disorders efficiently and effectively to improve clinical outcomes

Requirements:

  • Experience providing care to patients with mental health and substance use disorders
  • Case management experience preferred
  • Graduate of an accredited university with a master's degree in social work
  • Fully or provisionally licensed as a Licensed Clinical Social Worker without restrictions in the State of North Carolina
  • BLS Certification
  • Active North Carolina Driver License

  • Monday, Tuesday, Wednesday, Thursday, Friday
  • 8-hour shift, day shift / night shift (depending on the schedule of the clinic)
