Job Summary

Envision Home Health & Hospice is seeking a compassionate and skilled Hospice Nurse Case Manager to join our team. This role will provide high-quality care to terminally ill patients in various residential settings, including homes, assisted living facilities, and skilled nursing facilities.

Key Responsibilities
  • Assist the RN Case Manager with visits and provide nursing care to patients as needed.
  • Provide education, assistance, and support to patients and their families in the home care situation and during bereavement.
  • Regularly assess and reassess the nursing needs of patients and develop and re-evaluate care plans in conjunction with the Interdisciplinary Group (IDG).
  • Perform specific nursing procedures as needed, following doctor's orders, and maintain accurate records as required by Hospice.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in on-call system and provide on-call coverage when unavailable for assigned duties.
Requirements
  • Graduate from an accredited school of nursing.
  • Currently licensed as a registered nurse to practice in Washington state and must have up-to-date CPR certification.
  • Ability to work independently and make accurate, and at times, quick judgments.
  • Ability to respond appropriately to crises outside of a hospital setting.
  • Acceptance of and adaptability to different social, racial, cultural, and religious modes.
  • Completes Hospice training program.
  • Minimum 2 years of experience as a registered nurse, preferred.
  • Active patient contact within the past three years is preferred.
  • Must have a current valid driver's license, auto liability insurance, and reliable transportation.
