We are seeking a skilled Admissions RN to join our team at Envision Home Health & Hospice. As a key member of our interdisciplinary care team, you will be responsible for admitting new referrals into our care for Hospice patients.
Key Responsibilities- Coordinate all aspects of interdisciplinary care and services provided to hospice patients and their families.
- Provide education to families and caregivers on hospice care and services.
- Develop, implement, and review healthcare plans to ensure high-quality patient care.
- Advocate for patients and their families to ensure their needs are met.
- Provide direct patient care and document care provided.
- Manage symptoms and promote comfort and quality of life for patients.
- Current RN license to practice in the State of WA.
- At least one year of experience working with Hospice patients, preferably in a case management role.
- Current WA driver's license and reliable transportation.
- Maintain current CPR certification.
- Demonstrate excellent observation, problem-solving, verbal, and written communication skills, and nursing skills per the competency checklist.
- Show ability to organize and prioritize workload independently.
