Medical Care Liaison

The Oakland Community Health Network is seeking a highly skilled Medical Care Liaison to join our team. This role is responsible for ensuring seamless transitions between primary care and specialty care services.
The ideal candidate will have a master's degree in a mental health field and a current State of Michigan license as a Social Worker (LMSW), Counselor (LPC), or related field. Experience with Federally Qualified Health Centers is preferred.
Key Responsibilities:
- Evaluate patient needs and identify potential barriers to care.
- Develop and implement individualized plans to address these barriers.
- Collaborate with health center staff to ensure timely and effective transitions between primary care and specialty care services.
Requirements:
- Master's degree in a mental health field.
- Current State of Michigan license as a Social Worker (LMSW), Counselor (LPC), or related field.
- One year minimum of Behavioral Health experience.
- Experience with Federally Qualified Health Centers is preferred.
Salary: $70,000 - $90,000 per year, depending on experience. Benefits include medical, dental, and vision insurance, 401(k) plan, and paid time off.
