As a RN-Long Term Care Specialist, you will provide direct nursing care to patients in a long-term care setting, implementing and monitoring patient care plans, and serving as a primary coordinator of all disciplines for well-coordinated patient care.
Responsibilities- Provide direct nursing care to patients in accordance with established policies, procedures, and protocols of the healthcare organization.
- Implement and monitor patient care plans, ensuring the highest level of care and quality of life for our residents.
- Monitor, record, and communicate patient condition as appropriate, collaborating with the healthcare team to ensure seamless care.
- Assure resident safety by identifying safety hazards and emergency situations, and initiating corrective action as necessary.
- Assess and coordinate patient's discharge planning needs with members of the healthcare team.
- Licensure/Certification/Registration: BLS Provider obtained within 1 month of hire date or job transfer date required, American Heart Association or American Red Cross accepted.
- Registered Nurse obtained prior to hire date or job transfer date required.
- Education: Diploma from an accredited school/college of nursing or required professional licensure at time of hire.
