About the Role

We are seeking a skilled and compassionate Registered Nurse to join our team at Via Christi Village in Hays, KS. As a RN-Long Term Care Specialist, you will provide direct nursing care to patients in a long-term care setting, implementing and monitoring patient care plans, and serving as a primary coordinator of all disciplines for well-coordinated patient care.

Responsibilities
  • Provide direct nursing care to patients in accordance with established policies, procedures, and protocols of the healthcare organization.
  • Implement and monitor patient care plans, ensuring the highest level of care and quality of life for our residents.
  • Monitor, record, and communicate patient condition as appropriate, collaborating with the healthcare team to ensure seamless care.
  • Assure resident safety by identifying safety hazards and emergency situations, and initiating corrective action as necessary.
  • Assess and coordinate patient's discharge planning needs with members of the healthcare team.
Requirements
  • Licensure/Certification/Registration: BLS Provider obtained within 1 month of hire date or job transfer date required, American Heart Association or American Red Cross accepted.
  • Registered Nurse obtained prior to hire date or job transfer date required.
  • Education: Diploma from an accredited school/college of nursing or required professional licensure at time of hire.
