At Knife River Care Center, we are seeking a highly skilled and dedicated MDS Coordinator to join our team. As a key member of our interdisciplinary team, you will play a vital role in ensuring that our residents receive the highest level of care and support.
Key Responsibilities:- Coordinate with the interdisciplinary team to develop and implement care plans that meet the unique needs of each resident.
- Ensure that all necessary documentation is completed accurately and in a timely manner, including MDS, care plans, and CAA's.
- Review resident charts to ensure that all necessary information is documented and up-to-date.
- Collaborate with the DON, Business Office Manager, and interdisciplinary team to ensure seamless communication and coordination of care.
- Assist with the transmission of MDS data to CMS and State, and follow up to ensure acceptance and resolution of any issues.
- Participate in QAPI teams and nursing department meetings to ensure that our care is of the highest quality.
- Currently licensed as RN or LPN in the state of North Dakota.
- Must be IV certified or working towards IV certification.
- Hold current CPR certification.
