Job Overview:
WC Health is seeking a skilled Clinical Care Specialist to join our team. As a key member of our healthcare coordination team, you will play a vital role in ensuring that patients with chronic conditions receive the highest level of care and support.

About the Role:
This is a full-time position that requires excellent communication and organizational skills, as well as the ability to work effectively in a fast-paced environment. The ideal candidate will have experience working with Electronic Health Records (EHR) software and possess strong clinical knowledge and problem-solving skills.

Key Responsibilities:
- Complete individualized patient-centered care plans using our CCM software
- Manage frequent contact with patients to provide care coordination, support, and manage compliance with the Chronic Care Management program to increase positive outcomes
- Maintain ongoing communication with Primary Care or Psychiatric Clinicians regarding emergent patient needs and life-threatening episodes
- Document all client communications in an accurate, efficient, and timely manner

Requirements:
- 3-5 years of clinical experience (Medical Assistant, LPN, RN, or equivalent)
- Experience working with EHR software as a user
- Strong communication and organizational skills
- Ability to thrive in a fast-paced environment
