Nurse Care Specialist

About the Role:
We are seeking a skilled Registered Nurse to join our team in an outpatient dialysis center. As a Registered Nurse, you will be responsible for delivering high-quality patient care and working collaboratively with our interdisciplinary team to achieve excellent patient outcomes.
Responsibilities:
- Provide direct patient care and develop individualized treatment plans
- Collaborate with healthcare team members to ensure seamless care coordination
- Conduct patient assessments and monitor vital signs
- Administer medications and treatments as prescribed
- Document patient progress and communicate with healthcare team members
Requirements:
- Current Registered Nurse (RN) license in the state of practice
- Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree
- Experience in dialysis or nephrology nursing preferred
