The ALS Association is seeking a highly skilled and compassionate Care Services Coordinator to join our team. As a key member of our care services team, you will play a vital role in supporting individuals and families affected by ALS.
Key Responsibilities- Program Delivery and Measurement
- Develop and implement care plans to ensure individuals with ALS receive comprehensive support and services.
- Conduct home assessments and provide education to individuals and families on ALS-related topics.
- Collaborate with healthcare professionals and community resources to ensure seamless care coordination.
- Evaluate client needs and identify opportunities for improvement in care delivery.
- Support Groups
- Plan and facilitate monthly support groups for individuals and families affected by ALS.
- Develop educational content and coordinate with guest speakers to enhance support group experiences.
- Administrative Functions
- Ensure accurate and timely documentation of client information and care plans.
- Prepare reports and presentations to stakeholders, including the Care Services Director and other team members.
- Public Education and Awareness
- Develop and implement public education initiatives to raise awareness about ALS and The ALS Association's services.
- Collaborate with the Development Team to promote The ALS Association's events and programs.
- Bachelor's degree in a related field, such as public health, social work, or nursing.
- Minimum 3 years of experience in healthcare or community services, with a focus on care management and advocacy.
- Knowledge of Medicare, Medicaid, and patient assessment skills.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to work in a fast-paced environment and prioritize multiple tasks and deadlines.
- A competitive salary range of $27.99 per hour.
